Our Care Coordination Team will help you create and implement a comprehensive plan of care designed specifically for you and your loved one.

(a) It is not within the scope of the license of our agency to manage the medical and health conditions of clients should the conditions become unstable or unpredictable.
(b) Our employees are background checked and are required to take a minimum of 8 hours of training yearly
(c) The charges for the personal care services provided by the agency are below.
(d) Our Clients Rights and Responsibilities Form will be attached, along with our Grievance Form
