Position Summary
The LPN/RN Care Coordinator ? Phone Triage performs clinical specialty care coordination to our patients, providing health advice over the phone through compassion, empathy, and a strong desire to make a difference.
The Triage Nurse will assess and triage patient symptoms to assist in obtaining the appropriate level of care and/or self-care advice and documents calls as required.
An integral part of our care team, the Nurse Triage team is a trusted liaison for our patients, our providers, and our facility partners ensuring each patient feels comfortable and informed about the next steps in their care.
Responsibilities
Uphold and support the vision and values of Center for Urologic Care.
Interacts with outside organizations, physician practices, and other facilities to coordinate care of the patient in a professional, respectful manner.
Responds promptly to each incoming call.
Evaluates and assess patient?s health needs based on patient?s signs and symptoms utilizing established protocols, guidelines, and appropriate nursing services within the scope of licensure.
Obtains appropriate input and direction from the provider as needed to determine best clinical course of action and recommendations for specific health care needs.
Accurately and timely documents symptoms/complaints, nursing assessment, advice provided, and patient/caller response.
Coordinates and schedules same-day or near future appointments on the provider schedules.
Utilizes standing order and office protocols appropriately.
Provides patient education: (pre-op/post-op instructions, chronic conditions/diseases, preventative, medication, catheter teaching, etc.
).
Assures pertinent medical information is present in patient EMR (Urochart).
Assures the pertinent medical information is forwarded to proper departments as needed for continuity of care.
Manages prescription refills per office protocols.
Assists with prior authorizations of medications, testing, treatments, and durable medical supplies as needed.
Reviews patient results for abnormalities and inputs into EMR imaging, testing, labs, pathology, and microbiology results.
Obtains appropriate input and direction from the provider as needed to determine best clinical course of action specific to the patient.
As needed, performs other nursing functions as assigned.
Assist providers as needed.
Collaborates with variety of interdisciplinary departments within the organization and outside the organization to coordinate the care of the patient.
Serves as a clinical resource and role model for non-licensed members of the team.
Participates in performance improvement and personal development activities as assigned.
Works collaboratively with leadership team on strategies, ideas, and solutions towards improving processes, work flows, revising policies or procedures which can enhance patient care and/or the corporation.
Completes other duties as assigned by Administration.
